I have never had to use one of these on one of my planes but my observation is that their specification is a bit "casual", at least for the types of engines/props on 2-4-6 seat aircraft.

Does the engine or prop manufacturer actually test the combination with various prop hub extenders? Are there some guidelines somewhere?

I realize that with turboprops and large recips, prop whirl mode is a consideration and I would expect that this could also occur with smaller recips if the extension was long enough.

Any info appreciated.
